Self-Management Workshop

The Chronic Disease Self-Management Workshops are proven health and wellness workshops that complement provider care for adults with ongoing health conditions. The workshops are for individuals with chronic conditions and the support person that plays an active role in the participant's health management.

This workshop teaches adults strategies to empower and improve overall well-being. Self-management workshops complement clinical care and are evaluated and approved by the CDC. This workshop meets once a week for six weeks, for 2 to 2.5 hours.

Program Information

Standford University's widely tested Chronic Disease Self-Management Program helps those living with a chronic health condition improve their general health. A six week, once a week program, participants that attend the workshop will receive a free copy of the Living a Healthy Life with Chronic Condition book, and a relaxation CD, Relaxation for Mind and Body. 

This workshop is for:
  • Adults with chronic diseases {e.g. hypertension, arthritis, heart disease, stroke, lung disease, diabetes}
  • Family members, friends, caregivers of adults with chronic disease(s) 
This workshop teaches participants:
  • Techniques to deal with problems such as frustration, fatigue, isolation, and poor sleep
  • Exercise for maintaining and improving strength, flexibility, and endurance 
  • Proper nutrition
  • Appropriate use of medications
  • Communicating effectively with family, friends, and health professionals
Program components:
  • Structured, interactive, group learning focused on individuals suffering from chronic conditions.
  • Coping strategies: Action planning/feedback, behavior modeling, problem-solving, and decision making
  • Self-management of symptoms: Stress management/relaxation, healthy eating, sleep/fatigue management, medication management, exercise, communication with health providers
Program Outcomes:
  • Cost-savings: Lower healthcare costs
  • Increase health status: Less pain, fatigue, worry
  • Increase self-efficacy 
  • Increase communication with healthcare providers
  • Increase health status in 7 of 9 variables: Fatigue, shortness of breath, pain, social activity limitation, illness intrusiveness, depression, health distress
