To check out ultrasound equipment or to request practice of ultrasound at SimLife
Center, please fill out the following form. All equipment is available for 24 hour
checkout and is to be used for educational purposes only. EQUIPMENT IS NOT TO BE USED FOR DIAGNOSTICS. Patron is responsible for the equipment that is checked out.
Please allow 24 hours for the approval of the request. Approval is based upon prior
documented knowledge and skills of ultrasound technology.
If you will need space within the Simulation Center to use ultrasound equipment,
please fill out the following request in conjunction with this form: Simulation Center Room Reservation Form.
